My Brother is dying - quickly with no will?

He has suffered with Mental Illness his entire adult life going in and out of periods of Paranoia. He has a house, a boat, a substanial 401K - no wife, but one estranged adult daughter who hasn't talked to him in maybe 6 years. Because of his mental illness he refuses to sign his will and directives. What is going to happen to his belongings and money? We live in Pennsylvania. He may not last the week. His brothers and sisters have taken care of him for the past 20 years, but because of his mental illness and Paranoia, he feels that if he signs his will, we will no longer get him the medical care he needs. He is in Hospice now - there is no way to save his life at this point he's in the final stages of cancer.
