Who has the right to discharge my MOM from a nursing home who has cancer?

I'm her only next of kin, and she has no health directives or anyone at home to care for her. But, she said she'll be going home in a few weeks. Does the doctor order the discharge? She is in Stage 4 which has metastasized to her bones, nothing they can do. At least I know she's being cared for at the nursing home. I am handicapped myself, and cannot care for her.
